Internship Process
1. Application and issurance of provisional registration
- Eligible applicant will be requested to apply for Provisional Registration by the SLMC.
- A scan copy of the duly filled application form should be uploaded to the SLMC website.
- The physical copy should be handed over to the SLMC office.
- Provisional Registration certificate and the Identity Card is issued to the applicant.
2. Reporting to the Ministry of Health
- The applicant should report to the Ministry of Health who will allocate the Intern to the recommended hospitals.
- Applicant should report to the allocated hospital and contain the Internship slots.
- Applicant should submit duly filled form A to the SLMC with a copy to forms@slmc.gov.lk, within a week of accepting the first Internship slot.
- An e-mail notification will be sent to the applicant.
- If a confirmation e-mail is not reached within a week, the applicant could contact the SLMC on +94716355705.
- All forms should be clearly filled (written). All scanned copies should be clearly visible. unclear and illegible forms will have a chance of rejection.
3. Reporting of the evidences of Good conduct at the Internship
4. Completion of the allocated Internship
5. Application for Full Registration
- Duly completed following forms together with other required documents should be handover to the SLMC.
- Application for Full registration form (Form “F” to be converted).
- Upon completion registration certificate and the Identity Card is issued to the applicant.
