Notice to Dental Graduates Who Have Submitted Applications for Provisional Registration – 2026

Issuance of PR Certificates and Intern Identity Cards

Dental graduates who have submitted applications for Provisional Registration are informed that the issuance of Provisional Registration (PR) Certificates and Intern Identity Cards will take place as follows:

• Date: 30 July 2026 (Thursday)
• Time: 9.00 a.m. – 12.00 p.m.
• Venue: Sri Lanka Medical Council, No. 31, Norris Canal Road, Colombo 10

All applicants must be physically present to collect their certificates. Certificates will not be issued to third parties or nominees.

Please bring your National Identity Card (NIC) when collecting your PR Certificate and Intern Identity Card.

Internship Evaluation Book

The Evaluation Book (Evaluation Certificate for Internship) can be downloaded from the Evaluation Book – Intern Dental link.

Instructions for Printing Internship Forms

1. Download the latest updated internship forms from the SLMC official website. Old versions will not be accepted.
2. Print Form A as a single-page document, leaving the reverse side blank.
3. Each progress report must include the “Reported Commendable Events” page on the reverse side. Only the Progress Reports for OMF 1, OMF 2, Restorative Dentistry, and Orthodontics (SLMC Copy) must be submitted during registration.
4. Print Form C Part A and Part B on both sides of one sheet of paper. Print Part C as a single-page document, leaving the reverse side blank.
5. Form C Part A and Part B printed on separate sheets, or forms with alignment issues, will not be accepted.
6. Do not complete Part D of Form C, as it is for office use only.
7. Print Form D as a single-page document, leaving the reverse side blank.
8. Print the SLMC ID Card application as a single document, leaving the reverse side blank.

Important Note

Signatures and seals required for Form C:

• Part A: Consultant’s and Director’s signatures and seals
• Part B: Consultant’s and Director’s signatures and seals
• Part C: Consultant’s and Director’s signatures and seals

Ensure that the dates stated in Form C are correct and consistent, including the starting and ending dates.
