Replacement of damaged /misplaced ID card
Use this application to request a Sri Lanka Medical Council (SLMC) Identity Card. Please complete the application in block capital letters, provide the required supporting documents, photographs, and payment receipt for processing.
Certificate of Extract of Registration in place of damaged/ misplaced of Registration Certificate
If your original First Registration Certificate has been lost, you may request an official extract of your First Registration from the SLMC.
